Payment Information
The following paragraphs outline fee schedules and my policy regarding payments and cancellation of appointments made.
Types of Appointment
Initial evaluation includes a comprehensive intake of the presenting issue, medical history, a thorough nutritional, exercise and psychosocial assessment, and a physical examination. The minimum physician-patient contact time is 90 minutes.
Follow-up visits can vary in structure, frequency and duration depending on your individual needs, your medical provider infrastructure, and my recommendations of how to best achieve your goals.
The format is most often an outpatient office visit. I also offer home visits, community visits, family and group visits as convenient or germane to your treatment plan and with your agreement.
Fees
The cost for the initial evaluation is $250. This includes a written follow-up in the form of a nutritional and behavioral analysis and some coordination of prior medical care.
Follow-up appointment fees are $150 per 50-minute, $100 per 30-minute session. The fee covers analysis of any tests and coordination of care.
Payment and Insurance
Payment for services is required at the time of services rendered. I accept cash and personal or cashier’s checks, and money orders.
With each payment, I issue a receipt and statement detailing procedure/therapy (CPT) and diagnostic (ICD-9) codes commonly used in medical practice. You may submit these to your health insurance for reimbursements according to your plan(s) for out-of-network medical services.
Legal guardian(s) of minor patients are responsible for consent to treatment and payment of fees.
If you are of age and are insured with a parent or other insurance policy holder, I will issue the statement to you, and you may forward this as needed. Alternatively, you may give me written permission to share the statement, including diagnoses, directly with the policy holder you designate, for insurance purposes only. 
Lab Work and OtherTests
Any testing such as blood work, radiography, ECG, et al. that I may recommend as part of the assessment and plan may be done at your preferred facilities. These requests will be accompanied by diagnostic codes to facilitate coverage by your insurance(s).
Missed Appointments
I have a 48-hour notice appointment cancellation policy. This must occur by telephone.
If you do not cancel your appointment, you are responsible for the full fee.
If you cancel your appointment less than 48 hours before the scheduled appointment time, you are responsible for the full fee.
