    Your Initial Visit - Bring Your Forms and any Prior Tests

    Each package starts with a minimum one-hour initial visit. Filling out our patient forms and questionnaires prior to the appointment will allow us to focus on you, rather than paperwork, during the initial evaluation. You may send them by e-mail to drbailin@ownyourshape.com or simply bring them to your appointment. You can download our forms at this website, or request them to be mailed to you.

    If you have copies of any prior testing please bring those to your initial visit. Especially results from previous laboratory analyses, radiographic reports, any cardiac testing, immunization records, hospital records, etc., are informative and help prevent duplication of testing. We will discuss any such outside testing and its implications either at your initial office visit or, if received later, at the next follow-up visit.

    Integration with your Primary Care

    Your primary medical care needs will be integrated into your bariatric therapy. Many common conditions coexist with or have developed as complications of obesity such as diabetes, hypertension, elevated blood cholesterol level, arthritis, esophageal reflux, breathing problems, and others. Patients who already have a primary care provider may want to continue to see them for routine needs. With prior patient consent, we will gladly join efforts with your current health care professionals to coordinate our recommendations and improve the outcome for your overall health.

    Follow-up Visits and Your Ongoing Program

    Typically there will be regular follow-up visits in the office at specific time intervals. This varies with each package and the stage of your treatment. Some follow-up per telephone and electronically is optional and often integral to specific phases of ongoing therapy, especially for continued monitoring during more intense treatment periods.

    Intensity of therapy, i.e. length and frequency of office visits, is in part determined by your goals, and which package you choose. Ideally you will have a plan at the end of the initial consultation. During early phases of your treatment we will schedule more frequent visits in order to see if your regimen works and make adjustments if necessary. Any change in the intensity of your therapy can be made with the prior and mutual agreement between you and Dr. Bailin at any time. We are here to see you through to new healthier habits at a pace best suited to your needs.

    Preventive Screening and Coordination with other Specialists

    Preventive care and routine screening are integral to comprehensive medical care. It is particularly important to look for such disease markers early in the markedly overweight person, whose weight increases the risk of developing certain chronic diseases and malignancies, with a higher chance for disorders in multiple organ systems. Recommendations for such routine testing will be part of your specific treatment plan. Some conditions and complications, e.g. heart disease, severe degenerative joint disease, or preventive screening procedures, may necessitate referrals for a specialist opinion or additional testing.

    We work within a network of other medical specialists in surgery, cardiology, gastroenterology, gynecology, psychiatry, orthopedic surgery, neurology, and others. We may also draw on the expertise of allied health professionals, such as registered dieticians, exercise physiologists, and physical therapists, to devise the best treatment plan for you.
